The National Health Study offers an online application to create your own stickers and have them mailed to you. There is only one sticker template which is designed to promote the research program at Healthstudy.org.

Click the get started link and you can browse your computer for an image to upload. You can use a digital photograph or any other graphic that you have the right to use. If the file is too large you will get an error and will need to use a smaller file.
Once your picture is uploaded you can reposition it so it is centered on the sticker or choose another image. When you are satisfied with the image you can click continue and fill out your name and address. They ask for your email address but it is optional.
